IMO, transport probably didn't have anything to do with it.

Sounds like wiring/grounds.

On the fan coming on - swap relays, does the primary fan come on now? If so, may be a harness problem where it's grounding the relay. The CTS doesn't turn the fan on, it just sends cooling temp (voltage) signals to the ECM and the ECM thru Quad Driver circuit #3 does the actual grounding of the relay to turn the fan on. The power at the relays is always hot (key on)and the relays turn on when grounded.
No easy way to check the sensor without a Tech 1, jumping the harness won't display the 130+ degrees like it's supposed to with a Tech 1, IIRC.

You could pull the CTS and check it with an ohm meter just to see if there's current flow thru it.

Code 15 will also set if there's a harness problem so I'd look at the yellow & black wires from the CTS to the ECM.
