Stock style C4 T/O bearings are actuated bo a clutch fork, it's in contact with the throwout bearing that presses on the clutch plate. The fork sits on a pivot and goes thru a hole in the bellhousing where attaches to the slave cyl. The slave pushes the fork which pushes on the T/O beraing.

A hydraulic T/O bearing is a one piece assembly that uses the hydraulic pressure to push directly on the T/O bearing eliminating the slave and clutch fork.
