Check the throwout bearing for damage. It is basically a sandwich of two flanges and a collar, and has this stupidly thin metal core folded over the end and holds it all together. If it stretches the sandwich loosens up and can let the fork slip off.

To remove the fork, pull the pivot stud, you have to turn it clockwise to loosen it into the bellhousing. 12mm hex I think. Some cars require pulling the bellhousing to get the fork off the t/o bearing.

The clutch line could have went after the slave bottomed out and the pressure had nowhere to go.
